The term in bold commonly translated as 'church throughout all' is in Greek '”εκκλησια καθ᾽ ὁλης”—pronounced "Ekklesia Kath olos” or the Church Universal meaning the Catholic Church, since Kath olos is the same term used by St Ignatius of Antioch for the Church in his letter. The Roman world during the time of the Emperor Diocletian was a time of great persecution and threats of war. Power literally ruled the known civilized world.
Diocletian published his first edict against the Christians on 24 February 303 A.D. which outlawed outward forms of worship, principally the Mass. Many were martyred for their faith.
Near the year 300, St. Filomena (Philomena) was a young 13 year old girl whose parents were of nobility, St. Filomena was raised a princess influenced by her parents who after converting to the Catholic Faith, took a vow of virginity.
When Diocletian threatened war against Greece, St. Filomena's Father went to Rome to seek peace. Diocletian wanted too much, he fell in "love" with 13 year old Filomena wanting her for his wife. Because she refused, Diocletian subjected severe tortures and ultimately death upon St. Filomena. Her Story does not end there. Heaven intervened in such a powerful way.

1. In his saving plan, God wanted his only Son to be born of a virgin. This divine decision calls for a profound relationship between Mary's virginity and the Incarnation of the Word. "The eyes of faith can discover in the context of the whole of Revelation the mysterious reasons why God in his saving plan wanted his Son to be born of a virgin. These reasons touch both on the person of Christ and his redemptive mission and on the welcome Mary gave that mission on behalf of all men" (Catechism of the Catholic Church, n. 502).
